Administrative Forms

All users are requested to use the forms on this page for the respective purposes. All forms will be processed as soon as they are received. In order to facilitate processing, please kindly fill-in all necessary information. If you do not receive any reply from the System Administrator within five working days of your submission, please send a query to the Officer-in-charge to find out the status of your application.

 

Form Purpose
EGS-1

Use this form to request for the creation of new e-mail accounts. This request must be consented to by your respective controlling officer.

EGS-2

This form is meant for ministries, departments or agencies that wish to administer and mange their own domain names within the sabah.gov.my domain. They must also adhere to the electronic government domain policy.

EGS-3

Officers on transfer are encouraged to use this form to notify the System Administrator so as not to disrupt mail services for the officer concerned.

EGS-4

For other changes such as name of the account holder, promotion, change of division, telephone contacts, e-mail address, etc. should use this form.
